In addition, the client will be provided a resource binder on local agencies offering these services. Finally, the client will receive assistance in interviewing agencies and finding the best fit for them.
This package is perfect for clients who mainly need assistance with placement. We will discuss all the different senior living options, help determine which options suits your needs best, tour prep/companionship and a binder with details on local options.
This package is for the client who either wants to start planning their care or just recently became responsible for managing the care of a loved one. We’ll comprehensively explore all the issues of senior aging and create customized plan.

Consistent welfare checks and caregiver support you can rely on.
The Bridge Watch Program provides scheduled welfare check-ins (1-4 visits monthly) for your loved one. This service helps you feel confident that someone is regularly checking in, monitoring changes, and making sure your loved one is safe and supported
